RealTimeSync - Folder Compare

Hi,
When RealTimeSync is monitoring and detects changes, is it supposed to run through a full comparison of the folder pairs before syncing the changes?
The way I read the Manual, backed up by the Video, is that RealTimeSync will register to receive change notifications directly from the operating system in order to avoid the overhead of repeatedly polling for changes.
Am I reading this correctly? Any help would be appreciated.
