The fact that it worked for me, combined with the lack of support here would suggest that it may be a problem your end.
When you try and connect do you see anything in /var/log/auth.log?
I assume you've disabled any Windows firewall/HIPS you have running?

I haven't tried it and I don't know what ...

I just tried with Raspbian and 10.24 and it worked fine on port 2222.

If I try and connect on an incorrect port I receive the following message:

Given that your message is different, yet you've disabled the firewall (I'm not running one) and WinSCP is working I'm at a bit of a loss ...

I just tried from Windows 10 (FFS 10.25) to Raspbian and Ubuntu 18.04 and changing the port to 2222 worked fine for me.
I trust you restarted ssh?

Have you tried another program e.g. WinSCP?

I just had a quick look around and there is a sftp example on the libssh2 website that appears to implement TOTP (keyboard-interactive): https://www.libssh2.org/examples/sftp.html

Looking back at the forum post you linked to earlier it seems to be talking about an interaction between two clients ...

That's surprising, and a little annoying, that libssh2 is missing TOTP support!

In terms of your other points; you can configure the Google TOTP PAM to allow the reuse of TOTP tokens (within a valid time interval), and you can also change the number of time intervals for which a token is valid ...

Any news on a key that I can use to encrypt the server details, or somewhere I can securely send them?

I've repurposed a Pi for this testing so would be good to get this done sooner rather than later.

I just quickly tested setting up 2FA according to your link, but couldn't reproduce the login error.

That's strange, there's not a setting I should change for TOTP is there, as I'm only checking the Key file option and supplying the private key location?

Do you have a GPG key you could share ...

I'm afraid there isn't one to my knowledge. I could build a Pi and stick in on a vlan if you wanted something to test against, but that would take a little time. Are you saying that you think it should be working?
Is there nothing I can run this end for you?

I removed the TOTP requirement from the sshd config and FFS was able to connect successfully, but with it enabled FFS fails, whilst WinSCP and MobaXterm are fine.
I've currently set a Match block in the sshd config that requires only publickey authentication for sftp only accounts (to get it working ...

I'm currently using the Google Authenticator PAM module to enforce TOTP across a few of my servers, and I was wondering if you could support this?

When FFS encounters such a server the error from FFS is

Cannot find the following folders:

I've just tested it and the results look good! With compression off and 3 threads I'm now close to my Gigabit maximum.

60MB/s 1 parallel operation
110MB/s 3 parallel operation

To be honest I only use it once every few months and never looked into increasing the speed as it was always good enough ...

I've just tried, and turning compression support off at the server end brings the speeds back, so at least it can be controlled that way (assuming the user has that access).
Having looked around, the advice is that it probably will slow things down unless you're on a very slow network, so if you did ...

I did look at disabling compression on the server to test that, but ssh is my only access (short of carrying a monitor downstairs) and I didn't want to lock myself out :-)

I've just found and tested with the 10.24 beta and the speed is back, but only if I change the number of parallel file operations.

With v10.16, 1 parallel file operation and 10 SFTP channels I get 56MB/s
With v10.24, 1 parallel file operation and 10 SFTP channels I get 21MB/s
