On Linux Mint, the default install from the Mint software installer is the Flatpak version of your software.
Not knowing anything about programming, I'm reporting my findings.
FreeFileSync 12.1 installed via the Mint software installer loads the Flatpak edition of your software which must have some permissions or some other issue that causes RealTimeSync to fail 100% of the time. The installer loaded 12, and a manual run of flatpak updated the software to 12.1.
Mint is a new clean install.
Manual sync works fine.
Let me know if there is any other information you require.

Only the version available in the download section of freefilesync.org is getting official support. For all other versions, you will have to ask whoever created the package.
OK, that's unlikely. It's built by Flatpak and distributed by Linux Mint.
It's unfortunate, because you could lose a lot of potential paying donators. I paid for the donation version after thinking it might be faster, but for local disk i/o. it's not. Maybe over the Internet.
I almost gave up on freefilesync when I couldn't get it working and had no help with the default installer on Mint.

It wouldn't be worth the potential, and unlikely, extra money. The official documentation mentions exactly what the donation edition gets you and makes no claim to improve performance beyond what the hardware is capable of.
OK, thanks. When using Wifi + sftp I get better performance with it set to 4 threads (actual used was 3) than the default of 1 thread. 4mb/sec, almost 5 vs 2mb/sec roughly.

I have the exact same issue with the latest version from freefilesync.org running on the latest KDE neon version (fresh install from yesterday).
Manual sync on FFS works great! However, unfortunately not on RTS when opening and running the corresponding .ffs_batch file saved from FFS :-( No error message at all, neither on the GUI nor on the command line... RTS just simply does not detect any changes on the left-hand-side directory. FYI, I'm syncing between two local drives, on which I have full read/write permission.
What could be wrong? Can someone point me in the right direction? I cannot find any .ffs_batch or RTS log, is there one? I looked under ~/.config/FreeFileSync/ but couldn't find one there...

Then, I installed the one I downloaded from freefilesync.org.
Problem resolved.
If you're having problems with the freefilesync.org download version, then I have no clue how to help, sorry.

Yeah, actually, initially I installed the Flatpak version, that is what actually led me to this thread... then I decided to uninstall it and install the freefilesync.org version, as suggested here in the forum. But unfortunately no luck, the problem persists :-(
Let's see if eventually someone can shed some light upon this issue...
